一、准备资料及流程/ Preparation Materials and Procedures： 1.家长身份证复印件，在复印件上手写“我已知晓并同意 XXX 办理休学手续，签名和日期”； /A copy of the parent’s ID card,with the following handwritten note:“I have been informed and agree that[Student's Full Name]may proceed with the suspension procedures.”(Signed and dated by the parent) 2.完成本申请表上所有“意见”签字和背面“离校通知书”各部门签字盖章； /Complete all“Opinion”signatures on this application form,as well as the signatures and stamps of all departments on the reverse side“School Clearance Notice”. 3.完成资料准备和申请表签字盖章以后，到教务处明德楼808办公室开具休学证明。 /After preparing all documents and completing the signatures and stamps on the application form,proceed to Office 808,Mingde Building,the Academic Affairs Office,to obtain the suspension certificate. 二、学生本人描述申请事由/Application Reason (Written by the Student)： 学生本人签名/STUDENT SIGNATURE：__________________ 申请日期/APPLICATION DATE：__________________ |

SCHOOL CLEARANCE NOTICE

3、宿管中心（3、4号园区中间二楼）/DORMITORY MANAGEMENT CENTER(2ND FLOOR,BETWEEN ZONE 3&4)
4、国际学院（劝学楼1311室）/INTERNATIONAL COLLEGE(ROOM 1311,QUANXUE BUILDING)
请学生在上述部门办理完相关手续后，务必持《四川交通职业技术学院国际学生（学历生）休学申请表》与离校通知书，到教务处开具休学证明。否则后果自负！
After completing the relevant procedures at the departments mentioned above,students must bring the"SVTCC Application Form for Suspension of Studies(International Degree Students)"and the School Clearance Notice to the Academic Affairs Office to obtain the suspension certificate.Failure to comply may result in consequences borne by the student!
